If you wish to opt-out your child from the face mask policy, please fill out the ETA Mask Opt Out Form for Students below. Only parents who do not want their child to wear a mask should complete and submit this form. Parents must fill out a form for each child. Please return the completed form to the front desk or to your child’s teacher in their daily folder.
